q       HISTORY :

   Born to a Syrian father and a Belgian mother.  Completed secondary education at the American College in Aleppo in Syria in 1969, achieving the American High School Degree and passed the Syrian Baccalaureate Examination with distinction.  Studied medicine in the Medical Schools of Aleppo and Baghdad, and graduated from Baghdad Medical School in 1975 with distinction (ranked first in Surgery).  September, 1976 started  postgraduate training in psychiatry and enrolled in the three-year postgraduate course at Edinburgh University.  This was a course of three academic years part-time study combined with training at the Royal Edinburgh and associated hospitals.  Completed the course in October 1979, both in training and in study, and was granted the M.Phil. degree by thesis in psychiatry from University of Edinburgh in 1980.  Took up an appointment as Registrar at the Medical Research Council Neuro-psychiatry Research Laboratory at West Park Hospital, Epsom, Surrey, under the Directorship of Dr. Alec Coppen.  Appointed jointly in December 1980, by the South West Thames Regional Health Authority and the Medical Research Council to the post of Senior Registrar in Psychiatry and spent 18 months at the MRC Neuro-Psychiatry Laboratory until June 1982 when I moved into the St. George's Hospital Senior Registrar rotation.  Carried out research for the Ph.D. at MRC Neuropsychiatry Laboratory and Research Unit and the University of Surrey.  Awarded the Ph.D. in 1985.  Appointed Senior Lecturer at the University of Liverpool in October 1985.  Promoted to Reader in October 1992. Held the post of Professor and Chairman of the Department of Psychiatry and Behavioral Sciences in UAE University between December 1991 and March 1998.  Appointed Reader in Addictive Behavior – St George’s Hospital Medical School in April 1998. Married with two sons aged 17 and 15 years.  Interests include sports (squash, swimming) travel, music and poetry.

§          Principal Duties and Responsibilities:

Work with Faculty Committees, the Associate Dean for Undergraduate Medical Education and the Associate Dean for Medical Sciences, and with appropriate Faculty committees, including course Directors, develop Policies and Guidelines for Curricular Content related to the Clinical Sciences in the FMHS Curriculum.  Work with departmental chairmen to develop and recommend a budget and manpower projections for the support of academic and service operations within the Clinical Sciences departments, including teaching, research, and service needs.  Has overall responsibility for developing and supervising FMHS Internship Programs.  Work in cooperation with the chairmen of the clinical departments to develop strategic and long range plans for the further development of the individual departments and to determine facilities requirements for their operation.  Responsible for and supervise the internal and external Electives Programs of the FMHS.  Evaluate, with Dean, the Chairmen of the Clinical Departments.  Serve as the Chief Advisor to the Dean in all areas of Clinical Sciences operation and is a principal spokesman for the FMHS in matters relating to the Clinical Sciences with University and the Ministry of Health.  Chairman of, or serves on, appropriate FMHS Committees in the areas of responsibility delegated to me in this post.

§          Establishment of the Junior and Senior Rotations in Psychiatry with emphasis on student - based learning and problem solving.  Formal feedback from students indicated that the teaching was highly effective.

§          Introduction and co-ordination of a Course in Clinical Communication in the second year.  Evaluated as one of the most effective and enjoyable course in the Faculty.

§          Co-ordinator of the Neuroscience Module.  This is the largest module involving integrated pre-clinical and clinical topics.

§          Establishment of an Academic Case Conference for postgraduate training and continuing medical education.

q       ACHIEVEMENTS IN POST OF SENIOR LECTURER (1986 - 1991)

§          Re-organization of all undergraduate courses:

§          Introductory Course in the 2nd year became an intensive experience of psychiatric patients.

o         Three days block in the 4th year involved less didactic methods and an ethics debate.

o         One week lecture block in the final year: introduction of teaching of interview techniques by experience and example.  A novel method was developed with the assistance of Professor A. Cox, Professor of Child and Adolescent Psychiatry, which involves self-teaching of key interview skills in small groups.  Students undertake interviewing and active observation of colleagues with the aim of developing self-monitoring and appraisal.  Teaching of psychotherapy skills with Dr. Goepfert using an adapted package of the Grammar of Psychotherapy (J. Cobb and S. Lieberman).

 

§          Re-organization of Clerkship:

o         Introduction of Clinical Tutor's checklist to ensure that each student has experience of patients with a variety of disorders and experience of doctor-patient relationship (brief individual psychotherapy).

o         Introduction of a detailed feedback form to evaluate all aspects of the clerkship experience.

§          Teaching of specialties in smaller groups with more clinical experience.

§          Re-organization of the Postgraduate Course:

o         Postgraduate Course has become a three year rather than 2 year course, more research orientated to increase the uptake rate of the M.Psych.Med. by trainees at an earlier stage in the training.

o         A clinical and written examination was introduced in the end of the first year (M.Psych.Med. Preliminary Examination).

o         Introduction of Skills orientated course on psychotherapy using the St.George's model of the grammar of psychotherapy.

§          Re-organization of the weekly Academic Clinical Case Conference and Journal Club.  Improving the quality and methods of presentations.

o         Introduced a Monthly Academic Meeting for invited speakers.

o         Revision of the University's M.Psych.Med. regulations in liaison with the Higher Degrees Committee which enabled first year trainees to register for the degree.

o         Improvement of the training situation in the Professorial Units 1 and 2 by the amalgamation, of the two sub-schemes into one in relation to the General Rotational Training Scheme, enhancing the trainees' experience of out-patients, casualty and liaison and the attachment of one trainee to one consultant firm.

o         Introduction of Multidisciplinary Academic and Administrative Meetings at Sefton General Hospital.

o         Sponsorship of 10 overseas trainees from the Middle East via the W.H.O. and their supervision.

§          Compilation of a large series of Handouts for  Final Year medical students.

o         Lead the University of Liverpool's response to the M.R.C. Inter disciplinary Research Center initiative on neurodegenerative disorders.

o         The establishment of a Research Senior Registrar post in Biological Psychiatry on the Mersey Region Rotational Scheme.

q       CLINICAL DUTIES :

§          Liverpool

Honorary Consultant Psychiatrist at the Royal Liverpool Hospital with in-patients, day patients, outpatients and liaison commitments.

The clinical base for the Professorial Team (Professor J.R.M. Copeland, Dr. M.W. Forth and myself) (28 beds) is shared with two N.H.S. consultants.  I share responsibility for training two junior trainees, one full-time Senior Registrar and a clinical Lecturer.  The Senior Registrar has substantial involvement in liaison psychiatry with the Departments of Medicine, Genito Urinary Medicine and Dermatology.

A number of the allocated beds serve tertiary referrals of patients with resistant depression and anorexia nervosa (Regional service).  I also provide out-patient Regional assessment service for patients with eating disorders, jointly with Professor Peter Slade (Clinical Psychologist) and Dr. J. Hill (Senior Lecturer in Child and Adolescent Psychiatry).

From the 1st October, 1990, Consultant in Liaison Psychiatry.

I have set up a Consultation-Liaison Psychiatric Service with the University Department of Medicine.

UAE: Honorary consultant psychiatrist with inpatient duties at Al Ain Hospital and outpatient clinics at Tawam and Al Ain Hospitals.

        I lead two research groups:

 

§          Biological Psychiatry Group

o         This multi disciplinary group is concerned with biochemical, psychopharma-cological and neuroendocrine studies in depression.   The team includes:  Dr. A. George, Senior Lecturer in Pharmacology, Liverpool Polytechnic; Dr. K. Wilson and Dr. A. Anderson, Consultant Psychogeriatricians; Drs. Lovett and Upadhyaya, Lecturers in Psychiatry; Dr. T. Lock, Senior Registrar in Biological Psychiatry; Drs. R. Pathak, J. Collins and R. Burns, Research Senior Assistants in Psychopharmacology; Drs. H. Sajjad and M. Satish, Research Registrars; Mr. J. Swainston, Laboratory Technician; Miss. Suzanne Leigh, Research Psychologist.

 

o         The group's work follows on the work I carried out at the MRC Brain Metabolism Unit in Edinburgh and the MRC Neuropsychiatry Laboratory in Epsom, into the biology of depression.  My previous work has contributed to the establishment of peripheral biochemical markers of depression and the development of the dexamethasone suppression test (DST) as a useful aid in the management of depression culminating in a WHO World Collaborative Study of the DST, involving Centers in 12 Countries.  My work on lithium therapy has also established the minimal effective/safe dose for prophylaxis with the first double-blind trial of lower dose lithium as well as the determination of predictive clinical, psychological and biological factors of response to prophylactic lithium.  We have completed several studies on peripheral biochemical markers of depression and investigated their relationships to clinical response as well as investigations of hypothalamic pituitary function in depression in refinement of the existing neuroendocrine diagnostic tests.

 

§          Dementia Study Group

o         The group works in close collaboration with the Epidemiological Diagnostic Group funded by the MRC and directed by Professor J.R.M. Copeland and is concerned with planning and undertaking biological studies in dementia including neurophysiology (brain imaging, CT scan, SPECT and NMR Imaging and Spectroscopy), neuroendocrinology, neurochemistry and neuropathology.  A major aim is to determine ante-mortem markers for Alzheimer's disease by the study of early cases from the community study followed up to post-mortem.  Members of the group include:

o         The  group has completed studies using SPECT in dementia and depression of old age and is in the front-line of research on MRI image analysis and magnetic resonance spectroscopy in collaboration with Professor R. Edwards at the Magnetic Resonance Research Center.  The current work includes the investigation of physiological dysfunction and neuronal loss in healthy depressed and demented elderly using a non-invasive magnetic resonance image analysis technique.

 

§          Outcome of Depression in the Elderly Study Group

o         This is a newly formed group with the aim of undertaking short and long-term studies of outcome of depression in a community-based sample of elderly persons with case depression derived from MRC incidence study.  The study involves clinical, social, psychological and therapeutic studies on a cohort of 300 individuals.  Dr. K. Wilson will be chairing this group during my absence on sabbatical leave with the membership of Professor J.R.M. Copeland, Dr. P. Saunders and Mr. M. Dewey.

 

§          Molecular Biology Study Group

o         We have a developing collaborative group with Professor P. Rudland, Professor of Biochemistry in the University, with plans to investigate the molecular biology of dementia and the functional psychoses.  There is an ongoing collaborative study with the WHO biological section on the molecular biology of manic depressive illness.  We have identified large pedigrees of patients with functional psychosis for these studies.
